ESXi 5.5 host fails with PSOD when using Emulex elxnet version 10.4.255.13-1

The context is:

- ESXi 5.5 build 2638301

- hp BL460c-Gen8 2xIntel Xeon E5 2697-v2

- 128GBytes RAM

- FlexFabric CNA 554FLB partitioned NIC+FCoE connected to hp B22 (Cisco FEX - Nexus5548UP)

- FlexFabric CNA 554M in Mezzanine2 partitioned NIC+iSCSI+NIC+NIC connected to hp Virtual Connect Flex-10/10D (firmware 4.31)

- The hp FlexFabric CNA 554 is Emulex BE3 based with NIC function DeviceID 0x0710.

- The Emulex CNA NIC function firmware is version 10.4.255.25

Other Emulex drivers as iSCSI function driver (be2iscsi) and FCoE function driver (lpfc) are also version 10.4 but not offending.

The presence of Emulex NIC function driver (elxnet) version 10.4.255.13-1 in the context described above is causing PSOD.

The phenomenon occurs disregarding the version of be2iscsi and lpfc drivers version.

Attached are 4 files:

- PSOD screen capture as image file "150410-elxnet driver 10.4 PSOD.jpg".

- PSOD text OCR filtered in file "elxnet 10.4 PSOD.txt"

- Emulex controllers listing produced by "esxcli hardware pci list" in the file "pci enum Emulex.txt"

- useful outputs of esxcli commands before loading the elxnet 10.4 version, during the short while before PSOD and after rollback in the file ".ESXi 5.5 Emulex 10.4 driver issue.txt".
